Prior to First Appointment:
Intake are sent out online after an appointment has been confirmed from our facility. This intake allows that both our front office team and your physical therapist are fully prepared for your upcoming visit.
Intake are required 24-hours prior to your first appointment.

Preparing Your Child for Their Appointment
We understand that new experiences can feel intimidating, so we have designed our clinic to be welcoming and kid-friendly. To help your child prepare:
- Please have your child wear or bring comfortable clothing that allows for easy movement, such as athletic wear or loose-fitting pants, to ensure we have easy access to the injured area or any region of concern.
- Explain that the session will be fun and involve activities like games and exercises.
- Let them know they will be working with a friendly therapist focused on helping them feel better.
- Bring their favorite toy, book, or comfort item if it will help ease any nerves.

This means you can contact us directly to address your child’s musculoskeletal concerns, sports injuries, or other physical therapy needs.

Oregon is among the states that offer unrestricted Direct Access to physical therapy. This means no legal limitations prevent you from independently seeking physical therapy services.
